About The Position

The Resident Assessment Nurse (RAN) is responsible for conducting the Resident Assessment Instrument (RAI) process to ensure individual resident medical, psychosocial, emotional, and physical needs are identified, and care plans are developed to assist residents in achieving their highest practicable level of function. The RAN is an integral part of the facility team, expected to be a visible and engaged member, attending meetings, contributing to discussions, and upholding professional conduct. This role involves direct contact with residents, families, and the interdisciplinary team, requiring physical presence in the building.

Requirements

  • Registered Nurse (strongly preferred) or experienced LPN with a minimum one-year nursing experience in a skilled nursing/long-term care facility.
  • Excellent verbal and written communication skills with residents, families, all levels of staff, and external customers.
  • Proficient computer skills essential, including the ability to use Point Click Care and conduct online submissions.
  • Must possess the ability to work harmoniously with and supervise other personnel.
  • Must possess a high degree of personal accountability, responsibility, and the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures necessary for providing comprehensive activities to residents.

  • Prefer Resident Assessment Coordinator (RAC) Certification.

Responsibilities

  • Conduct required resident interviews (BIMS, PHQ-9, customary routines, pain assessment, interviews).
  • Perform Section GG functional observations and Section K nutritional assessments.
  • Gather, review, and synthesize supporting documentation across nursing, therapy, social services, and dietary.
  • Develop and maintain Care Area Assessments (CAAs) and comprehensive care plans.
  • Work in conjunction with the nursing team to ensure current resident care plans.
  • Participate in daily clinical huddles, IDT meetings, care conferences, Risk meetings, and family meetings, when necessary.
  • Coordinate with floor nursing on documentation needs that support accurate assessments.
  • Hand off completed clinical documentation to the central Resident Assessment Coordinator for technical coding and transmission.
