Under the direction of Evaluation leadership, this role involves working closely with program staff and leaders to identify opportunities for improvement and support the implementation of quality improvement projects. The associate will assist in developing evaluation plans, frameworks, and strategies that align with organizational goals and funder requirements. This position provides in-person support for implementing and sustaining new workflows, and supports data collection activities such as surveys, interviews, chart reviews, and focus groups, ensuring data quality and accuracy. The role includes engaging with patients/clients via telephone and in person to collect health indicator and behavior data, organizing community outreach efforts, and providing on-site coordination. Key responsibilities involve developing and refining performance indicators, targets, and data collection tools; analyzing and interpreting data to assess program performance and inform decision-making; designing and implementing Monitoring & Evaluation frameworks; collecting, analyzing, and interpreting data to track program effectiveness; producing periodic reports on KPIs and impact measurements; and maintaining accurate documentation in compliance with reporting standards. The associate will serve as a liaison between internal and external customers to improve patient/client quality of care and flow, and ensure documentation complies with grant funder and insurance requirements. Collaboration with departmental programs on retention and identifying barriers to care is also a part of this role. Additionally, the position involves creating standard and ad-hoc reports using Microsoft Excel, performing general clerical tasks, and handling confidential information.
