The Relief Pharmacist oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. They use effective patient education techniques to provide counseling to patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. The role involves designing, recommending, monitoring, and evaluating patient-specific pharmacotherapeutic regimens based on relevant biomedical literature, implementing recommendations effectively, and utilizing policies and procedures. This position also involves reviewing and accurately interpreting medication orders and prescriptions, participating in the management of medical emergencies, and actively supporting department medication safety and quality improvement activities, including event reporting. The Relief Pharmacist performs quality assurance and performance improvement of the medication use system, collaborates with the healthcare team to manage formulary adherence and the medication-use process, and assists in the acquisition of non-formulary, compassionate use, and investigational medications. Practice must be in compliance with organization’s policies and procedures, as well as local, state, and federal regulations. The role includes preparing, dispensing, and overseeing the proper storage and handling of medications. Additionally, the Relief Pharmacist serves as a preceptor for pharmacy students, residents, and healthcare learners, and mentors, orients, trains, and assists with performance evaluations of pharmacy teammates, residents, and students. They direct and check the accuracy of pharmacy support team members' work to ensure safe and efficient workflows, service standards, and regulatory compliance. The position requires the ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, understanding principles of growth and development over the life span and assessing data reflective of patient status to meet age-specific needs.
