The Rehabilitation Therapist (RT) position is primarily responsible for providing Targeted Case Management and Psychiatric Rehabilitation Services across all Strong Recovery clinics. This hybrid model of care is intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. The CCHBC RT will provide case management services to support patients around goals aimed at providing access to services that encourage individuals to resolve problems that interfere with their attainment or maintenance of independence, self-sufficiency, or maintaining themselves in the community. Additionally, the RT performs psychosocial rehabilitation services designed to assist the individual to overcome mental health and substance use barriers that may have interfered with the person’s ability to function independently and perform normative adult roles in the community. The intent of this role is to assist the individual to restore the individual’s functional level to the fullest possible. The primary focus of these services with the individuals includes assistance with vocational and educational goals. Under general direction and with considerable independence, performs several care management and psychiatric rehabilitation services consistent with the CCBHC and also the Department of Psychiatry Policy. Establishes and maintains cooperative working relationships with community providers and resources to obtain needed services and support for enrolled patients. Works as part of an interdisciplinary team to support the care of patients enrolled in a designated CCBHC outpatient clinic.
