We are mission driven, aiming always to empower, embrace differences and be BETTER TOGETHER in our approach to service more than 4,000 individuals throughout Massachusetts, and in areas of Connecticut, New York, Rhode Island, and Oklahoma. $500 sign on bonus ($250 at the completion of first week of training and $250 after successfully completion of first 90 days) Position: Rehabilitation Specialist – Full Time Location: Springfield, MA Hours: 35 hours per week Rate of Pay: $26.00 Per hour Schedule: Monday – Friday 8:30am –3:30pm Summary: Responsible for gathering data, documenting and presenting each member’s goals and objectives in accordance with ISP regulations; coordinating services with case managers and parent/guardians of program member’s goals and objectives, issues, concerns and abilities; and ensuring the provision of high quality services. The Rehabilitation Specialist is also responsible for overseeing the screening and prioritizing of referrals; documenting member progress; implementing behavior management strategies; investigating incidents and all other duties as assigned.

Requirements

  • Ability to use computers to include Microsoft Word, data entry, email and internet essential.
  • A minimum of 2 years of case management experience required.
  • Must be organized, detail-oriented, have solid time management skills and be able to work in a fast-paced multi-tasking environment.
  • Demonstrated ability to deliver high level of customer service/satisfaction with both internal and external customers.
  • Excellent communication, interpersonal and listening skills.
  • Must have a valid driver's license, good driving record, use of vehicle and willingness to transport participants in personal and/or agency vehicle.
