About The Position

The Rehabilitation Services Manager is responsible for the coordination and implementation of all rehabilitation services at the center and other participant locations, including assessments, treatments, equipment management, home modifications oversight. Ensuring coordination of care between PACE and contracted facilities, vendors, and providers. Responsible for departmental compliance with state and federal regulations, participation in organizational Quality Assurance program, departmental training, education, and coordination of rehabilitation services as part of the PACE IDT leadership structure.

Requirements

  • Master’s degree in Physical Therapy, or if applicant graduated prior to 1/1/2003, must possess a minimum of a Bachelor’s degree in Physical Therapy or a Bachelor’s degree and certification in Physical Therapy, or Master’s degree in Occupational Therapy, with a certification from the National Board for Certification in Occupational Therapy (NBCOT).
  • License in Physical Therapy Certification or Occupational Therapy Certification.
  • 2-3 years relevant experience working as an Occupational Therapist or Physical Therapist.
  • 1-2 years relevant experience in leadership and management of a team.
  • Must possess BLS/CPR certification or be able to obtain within 60 days of hire.

Responsibilities

  • Demonstrates effective leadership in managing daily department operations, managing staff, planning, and evaluating department activities to accomplish department objectives.
  • Develops, coordinates, and monitors department practice, standards, policies, and procedures based on established industry knowledge and research in accordance with all applicable federal, state, local, professional, and regulatory requirements.
  • Defines department quality objectives, identifies, and evaluates trends, implements courses of action, and evaluates outcomes.
  • Collaborates with the TRU PACE Operations Director in the annual and capital budget development and monitors departmental expenditures and maintains the departmental budget within its allowed and reasonable objectives as well as meet established budgetary guidelines.
  • Reviews and resolve quality issues and grievances timely in collaboration with QA department and Operations Director, recommends and implements system improvements as needed.
  • Ensures job responsibilities, authorities, and accountability of direct reports are defined and understood.
  • Accomplishes department objectives by communicating job expectations, planning, monitoring, and appraising outcomes.
  • Interviews, hires, trains, coaches, and provides constructive feedback and training opportunities to employees. Conducts employee disciplinary action in collaboration with supervisor and HR as needed.
  • Ensures completion initial and annual competencies timely, and provides direct oversight, to all department employees, contracted providers and others as assigned.
  • Develops annual department and individual employee goals, assesses employee performance, and develops employee performance improvement plans in conjunction with HR when needed.
  • Develops personal growth opportunities and facilitates team building for direct reports.
  • Holds employees accountable to company quality and customer service standards.
  • Participates as an active member of PACE IDT and TRU Community Care Leadership groups.
  • Participates as a member of the interdisciplinary team (IDT). Maintains regular attendance at, and participates in all IDT meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
  • Participates in the PACE QAPI program by identifying systemic opportunities for improvement, taking leadership of critical quality improvement initiatives, and contributing to process improvement projects.
  • Effectively identifies, responds to, recommends and/or resolves problems as they arise in collaboration with leadership team and IDT.
  • Takes lead in effectively communicating with IDT and community entities at meetings and throughout the day as needed.
  • Communicates effectively with the Operations Director and Medical Director to keep them abreast of any issues or concerns.
  • Develops and maintains effective work relationships with participants, families, peers, co-workers, contracted entities and vendors, and the public.
  • Protects privacy and maintains confidentiality of all company procedures and information about employees, participants, and families.
  • Follows all PACE Program Policies and Procedures and OSHA Safety Guidelines.
  • Demonstrates ability to coordinate multiple tasks simultaneously, and work independently with minimal supervision.
  • Maintains professional affiliations and any required certifications.
  • Manages flow of therapy services, oversees appointment scheduling, treatment plans in conjunction with RN Case Managers, primary care, and IDT. Ensures timely and appropriate care of participants.
  • Oversees and/or performs an in-person initial comprehensive occupational therapy assessment on all PACE participants upon enrollment and periodically as required by CMS PACE regulations and TRU PACE protocols.
  • Oversees and/or recommends a treatment plan to and coordinate with the IDT in development of the plan of care for each participant.
  • Evaluates participants’ needs. Follow up with participants during treatment. Discuss condition and treatment plans and recommend solutions to participant care problems with the IDT.
  • Oversees and ensures the planning of individual and group activities and implementation of treatment according to participant plans of care.
  • Oversees that therapy staff assemble appropriate equipment and data to structure an environment that will enhance participants’ responses to treatment.
  • Oversees therapy staff in the assessment of the home environment for safety and need for equipment and adaptations. Oversee the implementation and coordination of a home modification program, including installation of equipment and other changes to support independence and safety.
  • Oversees therapy staff in the re-evaluation of participant’s level of function to objectively determine participant’s response to treatment. Alter treatment plan and activities accordingly.
  • Oversees therapy staff in the education and instruction of participants and family members in proper care and use of various supports and exercise programs and in use of durable medical equipment in the home.
  • Ensures documentation of progress notes for each day of treatment including notations of the participant’s status regardless of whether significant changes have occurred. Document participant/family education.
  • Reviews, approves, and countersigns notes written by the Certified Occupational Therapy Assistant and Physical Therapy Assistant.
  • Exhibits communication skills necessary to interpret age specific responses to treatment.
  • Selectively delegates OT services and PT services to the Certified Occupational Therapy Assistant and Physical Therapy Assistant respectively as appropriate.
  • Trains PACE staff in methods to maximize safety and functional abilities in performing activities of daily living.
  • Maintains functional operating level of equipment and obtains approval for repairs and/or replacements as needed.
  • Inform supervisor of needs for equipment and materials for the department and for ppts.
  • Perform assigned duties as instructed for all emergency drills or situations. Maintain the department in compliance with safety regulations.
  • Create and manage authorizations for billing purposes for skilled rehab therapy and therapy provided in facilities.
  • Create and manage authorizations for billing purposes for contracted ST and DME that is needed and train other rehab staff in creating authorizations for DME, and home modification work contracted out.
  • Cover duties in the rehab department in terms of assessments, reassessments, treatments, and care coordination within the scope of the rehab supervisor’s authority to practice.
