About The Position

We provide rewarding and purposeful work. The Rehabilitation Services Floater provides support, assistance, and guidance to individuals with disabilities on the Mobile Crew, transportation routes, and Day Support programs in Prince William County as needed, helping them develop skills and engage with their community. Responsibilities include monitoring and supervising consumers, assisting with transitions, activities, and skill training, implementing ISP outcomes, providing personal care, collaborating with supervisors and staff, supervising Mobile Crew employees per contract requirements, and safely transporting consumers.

Requirements

  • High School Diploma or equivalent required.
  • One (1) year experience working with individuals with disabilities required.
  • Ability to use positive, proactive behavior support techniques.
  • Effective communication and relationship-building skills within a diverse workplace.
  • Must be eligible to work in the United States.
  • Be deemed eligible for hire by DBHDS and DSS in accordance with the criminal registry background check.
  • Present a positive and professional image to the public, coworkers, and Didlake consumers
  • Maintain insurability under Didlake's automobile policy. Requirements include being at least 25 years old and having at least 3 years of licensed driving experience.

Responsibilities

  • Provide direct support to consumers in accordance with written ISP outcomes and monitor and document daily progress toward goals.
  • Maintain and follow a daily schedule of training activities; assist with transitions, activities, skill training, and personal care.
  • Inform Program Manager of achievements, incidents, progress, or problems as needed.
  • Provide one-on-one support to consumers with additional needs, uphold Human Rights, complete required training, and support community activities such as fitness, recreation, and volunteerism in compliance with HCBS regulations.
  • Safely transport consumers to/from home, site, or workplace according to assigned routes and schedules.
  • Operate vehicles safely in accordance with state and local regulations and supervise consumers during transport.
  • Perform daily vehicle maintenance checks and record ridership, miles traveled, hours driven, fuel use, and maintenance needs.
  • Supervise employees by performing required elements of contract work and transporting them to designated worksites.
  • Assist with tools and supplies to complete tasks such as trash and recycling removal, while monitoring safety in parks and busy areas.
  • Ensure employees wear protective equipment and maintain all required safety gear, vehicles, equipment, and credentials.
