About The Position

We are seeking a Rehabilitation Counselor to work in our Behavioral Health Services Division. The role involves providing care and supervision of all assigned clients to ensure a safe, healthy, secure, and pleasant environment that promotes growth and development. The counselor will facilitate client growth and development toward independence in self-care, daily living skills, socialization, and community living. Participation in the development of individual service plans and related documentation is also a key aspect of the position.

Requirements

A.A. degree in the social sciences or health care related field and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
Good communication skills, both oral and written.
Ability to work independently and as part of a team.

Nice To Haves

May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs).
May require crisis intervention in emergencies.
May be required to report to work during emergencies including inclement weather.

Responsibilities

Provide guidance and support to all clients; teach and reinforce skill development in the areas of nutrition, basic self-care, routine medical care and follow-up, as developed through the joint planning sessions of the individual service plans.
Review and modify individual service plans to accommodate growth and development.
Ensure the overall safety of assigned individuals by maintaining a hazard-free environment and avoiding potentially dangerous situations.
Successful completion of medication training. Maintain and monitor medications in accordance with individuals’ self-medication plans, ensuring compliance with State and Agency medication administration regulations.
