About The Position

The primary purpose of the position is to serve eligible persons with significant disabilities in the 5 county-catchment area. The position is located in the Wilmington Independent Living Office. This position evaluates diagnostic data and determines eligibility, assesses client service needs, develops written plan of services in conjunction with the client and provides supportive guidance and counseling as well as monitors client progress. This position requires extensive travel to clients' homes in these areas. This position must maintain and develop resources/providers in coordinating services for clients. A positive working relationship with appropriate personnel in other agencies and community resources is required to provide a comprehensive plan of services.

Requirements

Master’s degree in rehabilitation counseling from an appropriately accredited institution; OR a master’s degree in a related human services field from an appropriately accredited institution; or current certification as a Certified Rehabilitation Counselor by the Commission of Rehabilitation Counselor Certification.
