About The Position

Lifepoint Rehabilitation is seeking a dedicated Rehabilitation Coordinator, Physical Therapist (PT) to join their team. This role is crucial in providing therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary environment. The position also involves contributing to program development, quality improvement, problem-solving, and productivity enhancement in a flexible, interdisciplinary manner. The primary customers are patients and clients, with a mission to build long-term relationships with client institutions and provide the highest quality of rehabilitation services.

Requirements

  • Graduate of a program in his/her professional discipline.
  • Hold an applicable and current license to practice.
  • Computer and current technology competence.

Nice To Haves

  • Experience working with an adult and geriatric population is preferred.
  • Experience serving people with physical disabilities is preferred.

Responsibilities

  • Coordinate the development of team members through structured activities.
  • Assist the Program Director in identifying educational needs of the team and implement corresponding in-service sessions.
  • Attend/present at unit in-service sessions as scheduled.
  • Attend external professional growth activities as scheduled and effectively communicate information to the team.
  • Encourage and assist with the development of student programs.
  • Assist the Program Director in providing team leadership.
  • Maintain open lines of communication between the team and the Program Director.
  • Assist Program Director with recruitment and interviewing process.
  • Assist Program Director in team member’s supervision and provide input into team member evaluations as requested.
  • Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others.
  • Adhere to applicable dress code.
  • Adjust personal schedule to meet needs of unit when requested.
  • Protect patient confidentiality at all times and facilitate effective communication with all patients.
  • Communicate professionally at all times.
  • Demonstrate and promote honesty and integrity at all times.
  • Promote adherence to policies and practices of applicable professional organizations and client hospital.
  • Be familiar with and act as a resource for team members regarding the contents of applicable professional code, company standard operating procedure/handbook, unit policy and procedure and hospital policies.
  • Contribute to client relations and marketing activities.
  • Assist the Program Director in establishing contact with discharge planners, physicians, and other referral sources.
  • Establish a positive relationship with facility personnel including administration, nursing, and therapy team members.
  • Monitor team members competency, licenses/certifications, and memberships in applicable professional organizations.
  • Ensure competency/skills assessment is satisfactorily completed annually by all team members and incumbent.
  • Ensure applicable licenses/certifications and memberships are current for all clinical team members.
  • Ensure incumbent license/certification and memberships are current and in good standing.
