About The Position
Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice. Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team, as appropriate to the role.
Requirements
- Current Basic Life Support (BLS) certification.
- Current State of Hawaii license in good standing appropriate to the discipline.
- Must arrive the Thursday before start in order to go to employee health appointment on Friday.
Nice To Haves
- Minimum two (2) years of experience in the SNF/ICF setting preferred.
Responsibilities
- Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life, as applicable to the discipline.
- Educate residents, families, and caregivers regarding treatment plans and patient progress.
- Complete accurate, timely, and compliant documentation.
- Document progress, changes in plan of care, and discharges according to facility, state, federal, and payer requirements.
- Supervise therapy assistants, when applicable to role.
- Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations.
