About The Position

The Rehab Nurse Manager at The Commons is responsible for the clinical leadership and daily operations of a high-acuity subacute/rehabilitation unit. This role oversees the care of medically complex residents requiring short-term, intensive nursing and therapy services with a focus on rapid recovery, safe discharge, and reduced hospitalization rates. Reporting to the Assistant Director of Nursing, the Rehab Nurse Manager plans, implements, and evaluates the effectiveness of nursing care delivered to the residents. The Rehab Nurse Manager uses management and administrative skills to provide the highest level of quality care and accomplish corporate, department and facility goals while adhering to established policies and procedures, as well as systemic clinical changes.

Requirements

  • 5+ years’ experience in nursing preferably in an acute/long-term care setting
  • 2+ years supervisory experience.
  • Degree from an accredited school of nursing.
  • Current NYS Nursing license and in good standing required.
  • Proven patient assessment skills (RN)
  • Current knowledge of best practice nursing standards and care.
  • Proficient in the care planning process.
  • Ability and knowledge to respond in emergency situations.
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
  • Fluency with English Language required (read, write & speak)
  • Ability to lead, guide and motivate staff.
  • Outstanding interpersonal skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Ability to manage multiple priorities in a busy environment.
  • Proficient in MS Office programs and able to learn new software rapidly.
  • An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

Responsibilities

  • Provide clinical oversight for high-acuity patients including post-surgical, orthopedic, cardiac, neurological, and medically complex cases.
  • Guide and support the team through high acuity admissions.
  • Lead interdisciplinary collaboration with therapy, physicians, case management, and pharmacy to drive outcomes and discharge planning.
  • Ensure timely admissions, assessments, and care plan development to meet regulatory and clinical standards.
  • Monitor and improve key quality metrics (rehospitalization rates, infection control, falls, LOS, functional outcomes).
  • Supervise, mentor, and develop nursing staff in a fast-paced, high-turnover environment.
  • Ensure accurate and timely documentation, including skilled documentation to support reimbursement (PDPM).
  • Lead performance improvement initiatives and regulatory readiness.
  • Manage the clinical care planning process to ensure the coordination and delivery of quality patient care. Review and update the care plan accordingly.
  • Provide or assist with direct care when needed, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated.
  • Ensure a safe and secure environment for clients/residents, staff and visitors.
  • Ensure residents/clients are assessed upon admission and periodically as conditions change. Conduct assessments as needed.
  • Obtain and implement Physician Orders, checking for accuracy and completeness.
  • Communicate effectively with the client/resident, family and interdisciplinary team and ensure staff members communicate effectively as well. Maintain the highest level of customer service within the area of responsibility.
  • Provide leadership and direction to personnel in accordance with organizational goals and mission.
  • Enforce the bargaining unit contract fairly and consistently. Address issues in a timely and appropriate manner.
  • Plan, implement and evaluate the clinical competencies and on-going educational needs of the staff. Update orientation or training materials as needed. Share current knowledge and up-to-date clinical expertise with staff through formal and informal education. Take responsibility for own education and stay clinically current.
  • Hold required monthly staff meetings across all shifts.
  • Mentor new employees and foster positive, collaborative relationships.
  • Ensure patient care delivered with best practice standards.
  • Develop, review and revise policies, procedures and other applicable documentation to ensure regulatory agency standards are met. Interpret policies, procedures and regulations for the staff.
  • Ensure clinical staff understand and comply with all policies, procedures and regulations.
  • Monitor adherence to the patient care standards through QAPI/QA processes. Actively involved in the development, implementation and evaluation of QAPI activities. Serve as an active member of the QAPI/QA committees.
  • In cooperation/collaboration with the QAPI/QA department, direct the data collection of all complaint investigations, incident reports and critical incident investigations. Summarize information and act after consultation with Director/QAPI/QA committee.
  • Review Point of Right to make improvements and changes in quality measures.
  • Keep Director informed of patient care problems and issues in a timely manner.
  • Manage unit expenses and costs (staff overtime, linens, supplies, etc.).
