Rehab Liaison - FT
About The Position
The Rehab Liaison will work with all appropriate members of the health care team to provide coordination of admissions and care through the continuum of services. This role ensures that time requirements for completion of Preadmission Screens are met, specific patient outcomes are achieved, and variances are evaluated and addressed. The position also ensures appropriate utilization of resources and services by reviewing admissions, duration of stay, and professional services rendered. The Rehab Liaison will consult with system staff (Administration, LSW, RN, MD) as appropriate for complicated issues, and market services to an assigned territory, meeting minimum monthly referral quotas. Duties include networking and travel up to 50% of the time, unit tours, representation at community outreach events, and physician networking.
Requirements
- 1 or more years of clinical experience as an RN, PT, OT, SPL, RCP.
- Current Clinical license in the state of SC (RN, PT, OT, SPL, RCP).
- BLS Required.
- Knowledge of Medical terminology.

- Bachelor’s degree preferred
