About The Position

Greenfield Rehab is seeking an experienced and dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Viroqua, WI. The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility. This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency. The Rehabilitation Director also functions as a licensed Physical Therapist or Physical Therapist Assistant, providing direct patient care as needed. We are looking for leaders who ensure the delivery of high-quality, patient-centered rehabilitation services, possess strong clinical reasoning and patient identification skills, are experienced managing medical complexity and changing functional status, are comfortable collaborating with nursing, therapy, and interdisciplinary teams, and encourage collaboration, value others' contributions, and build trust.

Requirements

  • Graduate from an accredited school with a minimum of a bachelor's degree in physical therapy (for PT).
  • Licensed by the State of Wisconsin to practice as a Physical Therapist (for PT).
  • 1 year experience as a treating clinician required (for PT).
  • Graduate from an accredited school with an associate degree as a Physical Therapist Assistant (for PTA).
  • Licensed by the State of Wisconsin to practice as a Physical Therapist Assistant (for PTA).
  • 1 year experience as a treating clinician required (for PTA).
