About The Position

Lakeview Health Services is seeking a dedicated and enthusiastic Rehab Coordinator to support individuals with mental illness and chemical addiction in achieving their goals and advancing their lives. This role involves a variety of administrative, coordinating, and monitoring functions within a residential setting. The ideal candidate will help create a healing environment that respects the perspectives and experiences of individuals, families, staff, and communities by practicing safe, respectful communication and respecting boundaries and differences.

Requirements

  • High School Diploma with five (5) years of relevant experience or an associate degree in human services or related field with three (3) years of experience or an equivalent combination of education and experience
  • One (1) year of residential experience preferred
  • NYS Driver’s License is required as driving is an essential function of this position
  • General knowledge and understanding of Mental Health issues, Chemical addiction and the Psychiatric Rehabilitation model
  • Knowledge of psychology, personality, and group dynamics
  • Knowledge of principles and practices of conflict and crisis management, adult learning, motivation, interpersonal communications, household management
  • Knowledge of independent living skills
  • Knowledge of the Human Services system
  • Knowledge of use of motivational interviewing to engage individuals
  • Knowledge of positive philosophy toward wellness/recovery and trauma informed care
  • Ability to use contemporary office equipment, particularly a computer with word processing, database and report generating software
  • Ability to communicate effectively with diverse individuals and to record notes as needed
  • Ability to listen, understand, and appreciate the experience of residents
  • Ability to establish rapport and meaningful professional relationships with residents
  • Ability to persuade, negotiate and resolve conflicts
  • Ability to work effectively under stress

Responsibilities

  • Assisting in providing information and site visit coordination
  • Monitoring medication management
  • Coordinating transportation
  • Completing household ordering and purchasing
  • Completing apartment/room inspections
  • Assisting in the orientation, training/education, and coordination of staff schedules
  • Assisting team members with service plan development and implementation
  • Conducting skills training in groups and individual settings utilizing evidence-based practices
  • Providing monitoring of medication oversight procedures; conduct weekly audits of the MAR book and provide results to the Program Manager
  • Conducting assessments and developing individualized services plans
  • May carry a caseload (residential program)
  • Developing and maintaining a structured daily schedule for clients based upon individual goals and preferences
  • Assisting residents in monitoring progress on stated goals and providing updates regularly
  • Providing individual counseling, training, and assistance
  • Monitoring systems around staff oversight of medications (residential program)
  • Encouraging and participating in social, recreational, educational, and cultural activities
  • Regularly assisting in general housekeeping and maintenance
  • Providing transportation as necessary
  • Maintaining group order in Residence and managing conflicts and crises appropriately
  • Maintaining complete and accurate documentation according to organization policies and procedures
  • Communicating with appropriate necessary service providers
  • Actively participating in required meetings, in-service trainings, and other continuing education opportunities
  • Knowing and adhering to all OMH/other agency regulatory policies and procedures
  • Managing an on-call phone in a rotation with Program Managers and other Rehab Coordinators
  • Supporting the program staff and residents in the absence of the Program Manager
  • Performing all other duties assigned by Program Manager or Director
