About The Position

Under the supervision of the PACE Therapy Supervisor, the PACE Rehab Aide provides support to the therapy team in the delivery of rehabilitation care to participants. This role performs non-medical tasks, such as setting up and cleaning treatment rooms and transporting participants to different areas of the health care facility.

Requirements

  • Must possess the skills necessary to provide care for geriatric participants and must be able to function well in a team environment.
  • Communicate effectively with participants and co-workers.
  • Work effectively in a team setting.
  • Patience and strong interpersonal skills.
  • Excellent oral, written, and computer skills.
  • Bilingual English/Spanish.
  • Ability to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team.
  • Valid California Driver’s License, acceptable driving record and vehicle insurance.
  • High school diploma or equivalent.
  • Six (6) months experience in a professional or personal care setting.
  • Current BLS CPR Card certified by the American Heart Association.
  • Minimum six months experience providing personal care to a frail, disabled or geriatric population.

Responsibilities

  • Assist with participant intake.
  • Greet and set up drop-ins on the machines.
  • Oversee drop-ins and monitor the gym in absence of, or when licensed staff is occupied.
  • Transport participants to and from the gym upon request of therapies staff.
  • Administrative support of the department, including scheduling of and follow up with participants.
  • Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed staff.
  • Scheduling and tracking of therapy department logistics.
  • Document participants' communication, responses and progress.
  • Liaise with physicians' offices and hospital personnel.
  • Responsible for preparation of equipment and supplies; and maintaining a clean and functional area for treatments and exercises.
  • Run and disseminate overdue/missing CPT reports for therapies weekly.
  • Daily completion of temperature logs.
  • Assist with therapy treatments on site and in homes including transfer, wheelchair follow during ambulation, etc.
  • Assist activities and day center as needed.
  • Assists in maintaining participant areas in a clean and orderly manner.
  • Cleans and organizes program areas including cleaning of equipment and shower areas.
  • Follow infectious disease precautions to prevent the spread of organism.
  • Follows established PACE and Day Center departmental and position specific policies and procedures and updates as needed.
  • Attends meetings, including interdisciplinary, and in-service educational training as required, and participates on committees as directed.
  • Maintains health and safety standards at all times, and takes an active role in the Safety and Injury in the Workplace Program; maintains all records and documentation as required.
  • Other projects and duties as assigned.
