About The Position

As a Social Worker / Dementia Care Clinician, you'll have the opportunity to provide meaningful clinical support that helps people navigate diagnosis, adjust to change, strengthen coping, and maintain quality of life throughout the dementia journey. This is a rewarding role for a Registered Social Worker who enjoys combining clinical practice with interdisciplinary collaboration. You'll work directly with individuals living with mild cognitive impairment or dementia and their care partners, providing psychosocial assessment, supportive counselling, care planning, education, advocacy, and system navigation. As part of your role, you'll also participate as a valued member of Primary Care Memory Clinic teams, collaborating with physicians, nurse practitioners, and other healthcare professionals to support early diagnosis and coordinated dementia care. Requirements

  • Bachelor of Social Work (BSW) or Master of Social Work (MSW) from an accredited university.
  • Current registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) as a Registered Social Worker (RSW).
  • Minimum three years of experience providing clinical social work services, preferably within healthcare, community health, or social service settings.
  • Experience conducting psychosocial assessments, supportive counselling, care planning, and case management.
  • Experience working with older adults, individuals living with cognitive impairment and their families.
  • Excellent communication, relationship-building, and organizational skills.
  • Valid Ontario Class G driver's licence and access to a reliable, insured vehicle.
  • Travel throughout the four-county region.
  • Vulnerable Sector Police Check (within the last 2 months).

Nice To Haves

  • An MSW and experience in dementia care, geriatrics, primary care, or community-based healthcare are considered strong assets.

Responsibilities

  • Provide comprehensive psychosocial assessments, supportive counselling, and therapeutic interventions for individuals living with dementia and their care partners.
  • Develop individualized care plans that promote wellbeing, safety, independence, and quality of life.
  • Provide case management and system navigation for individuals and families experiencing complex care needs.
  • Participate as a member of interdisciplinary Primary Care Memory Clinic teams, contributing to assessment, diagnosis, care planning, and coordinated dementia care.
  • Support families through adjustment to diagnosis, caregiver stress, grief, behavioural changes, family conflict, and future planning.
  • Facilitate psychoeducational support groups, including a once monthly evening online group, that promote caregiver resilience, dementia understanding, coping strategies, and social connection.
  • Collaborate with healthcare providers and community organizations to ensure coordinated, person-centred care.
  • Advocate for clients while connecting them with community resources and services.
  • Maintain professional clinical documentation and contribute to continuous quality improvement.
