Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. The RPN works as a member of the interprofessional team, applying a person-centered approach to care. This role involves completing and ensuring resident's clinical care assessments, care/service plans, and documentation are accurate and up-to-date. The RPN also implements and evaluates care plans, assists with scheduling resident care conferences, and facilitates resident move-ins, move-outs, and transfers. Direct resident care, medication/treatment administration, and processing orders for drugs and narcotics are key duties. The position also involves resident/family education, coordinating care plans with various stakeholders, and managing changes in resident condition or medical emergencies. The RPN teaches, delegates to, and directs unregulated care providers, responds to resident/family concerns, and follows infection prevention and control protocols. Upholding organizational values and completing other assigned duties are also part of the role.
