About The Position

The Village of Humber Heights has an exciting opportunity for a Registered Practical Nurse to join their team. This is a permanent part-time position with a bi-weekly commitment of 30 hours, typically working from 3pm to 11pm. The role involves providing high-quality personal care to residents, maintaining their well-being, and collaborating with a multidisciplinary team to ensure resident-centered care. Requirements

• Maintain current registration with the College of Nurses of Ontario.
• Existing Heart Save Level of CPR.
• Ability to prioritize tasks effectively, plan properly, and allocate appropriate time and effort to complete tasks.
• Ability to remain calm and manage stress appropriately.
• Ability to deliver safe administration of medication and treatments with a focus on accuracy, quality, and meeting residents’ quality of life requests.
• Ability to follow plans of care (or service plans) in accordance with the standards of the College of Nurses.
• Ability to identify and report any changes in resident well-being to the Charge Nurse or Village leadership team.
• Understanding of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector, including the Fixing Long-Term Care Act, 2021, Ontario Retirement Communities Association, and the Occupational Health & Safety Act (OSHA) of Ontario.
• Ability to support and embrace a resident-centred social culture and deliver high-quality personal care.
• Ability to work as a key member of a multidisciplinary team to understand residents’ individual needs.
• Ability to advocate for residents to actively participate in decisions about their care.
• Ability to support new resident move-ins, facilitate transfers to hospital, or manage changes in resident medical needs.
• Ability to collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
• Proficient communication and computer skills for updating care plans in the current system.
• Understanding of the physical, mental, and emotional demands of the RPN role.
• Proven alignment with organizational vision, mission, and values.

Nice To Haves

• MDS credentials.
• Experience working with people living with Dementia.
• Knowledge of the palliative assessments scale.
• Comfortable supporting residents and their families during end-of-life care.
• Knowledge of relevant policies, procedures, legislation, regulations and best practices applicable to the retirement and long-term care sector (e.g. RAI processes and CIHI, supplies, equipment ordering, etc.)

Responsibilities
