About The Position

Acclaim Health announces the availability of One (1) Established Part Time Registered Practical Nurse position to work in all of Halton Community and Clinics with a focus on North Halton. This position requires the individual to be available to work 3 shifts per week and 1:3 weekends in addition to the statutory holiday rotation.

Requirements

Minimum 1 to 2 years clinical practice in a community and clinic environment preferred
Current registration with the CNO
Current CPR certificate
Knowledge and experience in wound care (complex and non-complex), negative pressure wound treatment skill, IV and pump skill, and CVAD skill.
Possesses catheter care and maintenance skills.
Ability to administer medications
Excellent interpersonal, communication and teaching skills
Demonstrated initiative and commitment to professional development and skill enhancement
Valid driver’s license and access to a vehicle

Palliative experience is an asset.

Responsibilities

Provide nursing care to individuals and families in the Halton region and clinics, with a focus on the North Halton.
Organizes the day, set priorities based on continuity of care, the type of care to be given and the client’s/family’s health needs.
Develops, implements, evaluates, and modifies the plan of care outlining the strategies, orders and actions to achieve expected/desired outcomes.
Assesses the health status of the client.
Administers IV therapy including central line and peripheral line maintenance.
Administers medications topically through natural body orifices, through tubes and by injections.
Handles wound care, acute and chronic, for certain types and stages of wounds.
Responsible for ensuring that the laptop is synchronized with the network at a minimum of twice a day, at the beginning and end of your shift, in order to receive information on and update client records.
Check voicemails and fax mail on days scheduled to work.
Attends team/POD meetings as required.
