Reporting to the Director of Care (DOC), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. The RPN will work as a member of the interprofessional team, applying a person-centered approach to care. This role involves completing and ensuring resident's clinical care assessments, care/service plans, and documentation are accurate and up-to-date, and communicating changes as required. The RPN will also implement and evaluate the effectiveness of care/service plans, assist with scheduling and participating in resident's care conferences, and work closely with the DOC/DHW to facilitate resident transitions. Direct resident care, medication/treatment administration, and documentation are key components, adhering to regulatory and Extendicare standards. The role also includes processing orders, safe-keeping of drugs and narcotics, and facilitating resident/family education. The RPN will coordinate nursing team assignments for efficient service delivery, respond to significant changes in resident condition or medical emergencies, and provide emergency first aid. Teaching, delegating to, and directing unregulated care providers is also a responsibility. The RPN will address resident/family concerns, follow and assist in coordinating infection prevention and control, health and safety, and quality/safety plans. Upholding the organization's values and philosophy, particularly regarding ethics, morality, and integrity, is essential. Other duties as assigned will also be part of the role.
