About The Position

Sherwood Park Manor Long Term Care Home is a full-service non-profit Nursing Home licensed by the Ontario Ministry of Health and Long-Term Care and governed by a volunteer Board of Directors. Reporting directly to the Director of Care/Associate Director of Care, the Registered Practical Nurse (RPN) assumes responsibility as a team leader for an assigned unit, under the direction of the Registered Nurse. Primary duties include the safe provision of medications and treatments to residents.

Requirements

  • Registered Practical Nurse in good standing with the College of Nurses of Ontario.
  • Current CPR and First Aid certification.
  • Ability to supervise and oversee Personal Support Workers (PSWs) in a Team Lead role.
  • Maintain professional collaborative relationships with other multidisciplinary team members AND therapeutic nurse-client relationship with the resident and their family.
  • Demonstrate sound leadership skills in problem solving and decision-making.
  • Demonstrate effective, professional oral and written communication.
  • The ability to manage emergency situations effectively and efficiently.
  • Have the capacity to deal with distractions and multiple demands on one’s time.
  • The maturity to assume accountability for one’s own actions.
  • Must have an up to date 2 –step TB test.

Nice To Haves

  • Experience working in Long Term Care desirable.
  • Experience using PointClickCare an asset.
  • Act as an effective mentor to new RPN and PSW staff and provide ongoing encouragement and support as needed.

Responsibilities

  • Perform nursing skills in compliance with the College of Nurses of Ontario Standards of Practice
  • Take responsibility for the management of the nursing unit on a tour of duty under the direction of the Registered Nurse.
  • Assist with implementation of prescribed medical regime as per the resident’s POC.
  • Take responsibility for activities delegated by the Registered Nurse.
  • Continuously observe and update the assessment of the resident by using critical nursing assessment skills and all available resources.
  • Contribute to the development, modification and implementation of the resident’s Plan of Care.
  • Utilize all available resources to evaluate the effectiveness of nursing care given and revise the POC in consultation with the registered nurse.
  • Maintain records for each resident in compliance with policies of Sherwood Park Manor, documentation standards, and current legislation.
  • Provide effective support and supervision for non-registered staff and students
  • Perform additional related duties as required
