About The Position

The Village at St. Clair has an exciting opportunity for a Registered Practical Nurse (RPN) to join their team in a long-term care setting. This role focuses on providing safe, quality care to residents, maintaining their well-being, and supporting a resident-centered social culture. The RPN will work collaboratively with a multidisciplinary team, including physicians, to ensure residents' individual needs are met and they are supported in decisions about their care. The position emphasizes communication, computer skills for updating care plans, and understanding the demands of the RPN role. Requirements

  • Maintain current registration with the College of Nurses of Ontario.
  • Have existing Heart Save Level of CPR.
  • Demonstrate an ability to prioritize tasks effectively, properly plan for, and allocate the right amount of time and effort to complete tasks. Remains calm and manages stress appropriately.
  • Have an understanding of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector, including the Fixing Long-Term Care Act, 2021, and the Occupational Health & Safety Act (OSHA) of Ontario.
  • Proficient communication and computer skills in order to update care plans in the current system.
  • Understand the physical, mental, and emotional demands of the RPN role.

  • MDS credentials.
  • Experience working with people living with Dementia.
  • Knowledge of the palliative assessments scale.
  • Comfortable supporting residents and their families during end-of-life care.
  • Knowledge of relevant policies, procedures, legislation, regulations and best practices applicable to the retirement and long-term care sector (e.g. RAI processes and CIHI, supplies, equipment ordering, etc.)

Responsibilities

  • Maintain current registration with the College of Nurses of Ontario.
  • Deliver safe administration of medication and treatments with a focus on accuracy, quality and meeting our residents’ quality of life requests.
  • Follow plans of care (or service plans) in accordance with the standards of the College of Nurses and able to identify and report any changes in the resident’s well-being to the Charge Nurse or Village leadership team.
  • Support and embrace our resident-centered social culture and be proud to deliver the highest quality of personal care to our residents.
  • Work as a key member of our Village and Neighbourhood multidisciplinary team to understand residents’ individual needs, and advocate for residents to actively participate in decisions about their care, support new resident move-ins, facilitate transfers to hospital, or any changes in resident medical needs.
  • Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
  • Update care plans in the current system using proficient communication and computer skills.
