About The Position

We are looking for a compassionate RN to provide medical support and complete assessments for clients in their homes. The role involves one-on-one home visits, educating patients and families, communicating with care teams, assisting with new employee training, and supporting administrative staff with employee and client compliance. The successful candidate has a can-do attitude, a friendly demeanor, and excellent interpersonal skills.

Requirements

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing.
• Current state license as an RN.
• Proof of current CPR certification.
• Compassionate, friendly demeanor.
• Excellent communication skills.
• Current driver’s license and reliable transportation.
• Ability to work a flexible schedule and travel long distances.

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify home care clients’ physical, psychosocial, and environmental needs.
• Educate patients and their families on home care strategies and procedures, and recommend devices or tools that may improve patients’ quality of life.
• Communicate effectively with patients and clients, their family members, team members, and other health care professionals.
• Assist with training new employees.
• Assist administrative staff with ensuring employee and client compliance.
