Registered Nurse RN / Hospice
About The Position
Transitions Hospice is seeking a Registered Nurse to provide compassionate hospice care in the Mattoon, Charleston, and surrounding areas. Hospice care is delivered where patients feel safest—at home—and supports their physical, emotional, and spiritual needs. The nurse serves as a clinical anchor for patients and families, coordinating care, guiding the interdisciplinary team, and supporting patients through end-of-life care. The schedule is 8 AM–8 PM, with alternating four-day and three-day workweeks, including weekends in rotation. The role participates in an on-call rotation per team policy.
Requirements
- Active RN license in the state of employment.
- Graduate of an accredited school of Nursing.
- Compassion for patients and families navigating end-of-life care.
- Strong clinical judgment and ability to work independently in the field.
- Excellent communication skills with care teams and families.
Nice To Haves
- One or more years of experience in hospice, home health, or a related organization is preferred.
Responsibilities
- Conduct comprehensive and ongoing assessments of patients’ evolving needs.
- Develop and adjust personalized care plans in collaboration with physicians and care teams.
- Administer medications and treatments according to physician orders.
- Maintain accurate, timely, and meaningful clinical documentation.
- Communicate with families, physicians, and community health partners.
- Educate patients and caregivers to help them feel confident and supported.
- Participate in the on-call rotation per team policy.
