About The Position

The Registered Nurse works with the Health and Nutrition team and other CII staff in facilitating the administration of health services and programs for students and families. Provides training and technical assistance with health services mandates and programs including Federal and State guidelines. Requirements

  • Bachelor's degree in Nursing from a four-year university required.
  • 3 years of nursing experience required; preferably with young children ages from 0-5 .
  • Possesses a valid driver's license and state-required auto insurance or reliable access to transportation depending on business needs.
  • Required to travel extensively (> 50% of the time)
  • Ability to work with minimal supervision.
  • Ability to handle confidential information.
  • Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
  • Sensitivity to service population’s cultural and socioeconomic characteristics.
  • Excellent verbal and written communication skills.
  • Understands CII’s basic philosophy and participates fully in carrying out its mission.
  • Current valid California Registered Nurse’s License
  • Must have intermediate knowledge of Microsoft Office 365 (i.e., Word, Excel, Teams, etc.)

Responsibilities

  • Determines child health status by reviewing health documents, including health history, physical exams, dental exams, laboratory results and immunizations status for children birth to five as well as reviews health documents for pregnant persons. Shares the results of the determination with designated staff and parents.
  • Conducts home visits to discuss health needs, provides health education and early intervention with the child’s parents, pregnant persons.
  • Assists with conducting screenings such as - vision, hearing, TB assessment, hgb.
  • Documents and tracks the health status for children and completes follow-up to ensure each child is up to date and on a schedule of well childcare as outlined in the EPSDT guidelines. Provides periodic updates/progress reports.
  • Monitors database to ensure all children remain up to date on the schedule of well childcare.
  • Assists with planning and follow-up in a timely manner in accordance with timelines established by the agency, with health concerns, questions or abnormal findings that are brought forward by others.
  • Conducts ongoing monitoring of health requirements based on funder and CII requirements.
  • In collaboration with appropriate CII staff, maintains and monitors health files.
  • Provides related training and education to staff, families and community members.
  • Participates in Individualized Family Service Plans and Individualized Education Plans meetings as needed.
  • Participated in Multi-Disciplinary team meetings, as needed.
  • Participates in planning and/or convening the Health Services Advisory Committee.
  • Participates in self-assessment and community assessment as it is related to health services.
  • Works with enrollment and care coordination staff as needed with attendance related health issues.
  • Stays current with state, local public health and community health guidelines.
  • Other duties and special projects as assigned.
