About The Position

This position involves managing patient referrals for skilled home care services. The Registered Nurse will work with physicians, patients, families, and discharge planners to create and implement home care plans. The role is based in a Garden City office with a Monday-Friday schedule from 9:30 AM to 5:30 PM, including a weekend rotation. The position requires 100% on-site presence.

Requirements

• Graduate from an accredited School of Nursing.
• Current License to practice as a Registered Professional Nurse in New York State required.
• Must be enrolled in an accredited BSN program within two (2) years of job entry date.
• Must obtain a BSN Degree within five (5) years of job entry date.
• Prior years of inpatient acute patient care experience strongly preferred.

Nice To Haves

• Bachelor’s Degree in Nursing, preferred.
• Specialized certifications as needed.

Responsibilities

• Respond to referrals in a timely manner and evaluate patients for appropriateness of skilled home care services.
• Plan for the skilled home care needs of patients in a timely manner to ensure patient’s expeditious movement to the appropriate level of care.
• Implement a comprehensive home care plan based on the in-depth assessment of the patient’s needs, including coordination of activities with family, informal supports, and other community resources.
• Maintain working knowledge of third-party reimbursement guidelines and regulation standards.
• Function as a home care service resource person.
• Perform job functions in a fiscally responsible manner consistent with the mission and vision.
• Assist with orienting new nursing personnel to department policies, procedures, and practices.
