Farr 8 is a Medical-Surgical unit specializing in the care of kidney, liver, and pancreas transplant patients throughout the perioperative and postoperative phases of their transplant journey. The patient population is highly acute and often medically complex, with common diagnoses including End-Stage Renal Disease (ESRD) and End-Stage Liver Disease (ESLD). Nurses on Farr 8 provide comprehensive care that includes the administration of blood products, high-risk intravenous medications, and immunosuppressive therapies. The team also plays a critical role in patient and family education, including post-transplant care, graft monitoring, drain management, medication adherence, and lifestyle modifications necessary for long-term transplant success. This specialized unit offers a unique opportunity to care for a complex patient population while developing expertise in transplant nursing and multidisciplinary care coordination. The Clinical Nurse II utilizes the nursing process as the frame of reference for practice as a professional registered nurse and provides direct nursing care to patients and families.
