About The Position
As a Home Health RN, you’ll bring skilled, compassionate care directly to patients by performing patient assessments, admissions, and ongoing care planning. You will coordinate care with physicians and interdisciplinary teams, deliver therapeutic, preventative, and rehabilitative nursing care, and monitor patient progress and updating plans of care. You will also support patients and families through all stages of care and maintain accurate and timely documentation.
Requirements
- Active Registered Nurse (RN) license
- Graduate of an accredited nursing program
- Minimum 1 year of clinical experience (ICU, Med-Surg, or Rehab preferred)
- Current CPR certification
- Reliable transportation, valid driver’s license, and auto insurance
