About The Position

Havenova Home Care Solutions is hiring a Home Care Field RN in Massachusetts, with openings for both full-time and per diem (PRN) positions. They are a Massachusetts-based home care agency dedicated to delivering high-quality, personalized care to clients in the comfort of their own homes. Their patients include elderly adults, individuals with chronic conditions, and post-surgical patients. They are growing their clinical team and want nurses who bring both skill and heart to every visit.

Requirements

  • Active, unrestricted RN license — Massachusetts Board of Registration in Nursing (no encumbrances)
  • Current CPR / BLS certification (AHA or Red Cross)
  • Negative TB test (PPD or QuantiFERON) within past 12 months
  • Clear criminal background check (CORI) and OIG/SAM exclusion clearance
  • Associate Degree in Nursing (ADN) required
  • Minimum 1 year RN experience
  • Strong English communication skills (verbal and written)
  • Valid Massachusetts driver's license and reliable personal vehicle with current insurance
  • Ability to travel to client homes throughout the service area
  • Comfortable working independently and managing your own schedule

Nice To Haves

  • BSN strongly preferred
  • Home health, med-surg, or geriatric experience a plus
  • OASIS experience preferred
  • BSN or higher
  • Home health or hospice nursing experience
  • Specialty certification (CHPN, COS-C, or similar)
  • Bilingual skills (Spanish or other languages)
  • Experience with wound care, IV infusion, or complex medication management

Responsibilities

  • Conduct comprehensive patient assessments at start of care and on an ongoing basis
  • Develop, implement, and update individualized care plans per physician orders
  • Administer medications, IV therapy, wound care, catheter care, and other skilled nursing interventions
  • Monitor patient progress and respond promptly to changes in condition
  • Educate patients and families on diagnosis, medications, safety, and self-care
  • Coordinate care with physicians, therapists, and the home care team
  • Complete clinical documentation accurately and on time
  • Supervise and guide Home Health Aides (HHAs) and CNAs as needed
  • Participate in OASIS assessments, care conferences, and discharge planning
  • Maintain full compliance with HIPAA, infection control, and MA state regulations
