About The Position

Elara Caring is seeking a compassionate Registered Nurse for home health care. The role contributes to the delivery of care in patients’ homes by assessing patient needs, providing nursing care under plans of care and physician orders, and supporting safe, continuous, high-quality care. The nurse protects patient and staff privacy in accordance with HIPAA, monitors patient conditions, reports changes to the care team, educates patients and families, and takes appropriate action when a patient’s condition changes.

Requirements

• Hold a current State license as a Registered Nurse.
• Graduate from an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Have at least 1 year of experience as a nurse in a clinical care setting.
• Frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Have a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
