About The Position

New Dimensions Home Health Care is expanding its services and is seeking a Registered Nurse (RN) to join their home health team in the Fergus Falls area. This role is ideal for a motivated RN who values independence, flexibility, and providing high-quality, person-centered care. The RN will have the opportunity to build their own schedule while working directly with clients in their homes, helping them achieve the best possible outcomes in a comfortable, familiar environment. Training is provided for the right candidate.

Requirements

• Active Registered Nurse (RN) license required
• Strong assessment and clinical judgment skills
• Highly organized and detail-oriented
• Ability to work independently in the field
• Strong communication and interpersonal skills
• Compassionate, patient-focused approach to care

Responsibilities

• Perform INR draws and medication management support
• Set up and assist with medication organization
• Serve as a liaison between clients and physicians
• Complete patient assessments and accurate documentation
• Supervise Personal Care Assistants (PCAs)
• Support coordinated, patient-centered care plans
• Ensure continuity of care and communication across providers
