The Registered Nurse (RN) provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant’s condition to primary caregivers, physician and care team. This role functions as a member of the Interdisciplinary Team, maintaining regular attendance and participation in team meetings, communicating participant changes, collaborating on plan of care decisions and coordination for twenty-four (24) hour care delivery. The RN coordinates with other clinical staff to develop and implement a plan of care to meet participant needs for safe community living, aligning with their goals of care. They conduct comprehensive initial nursing assessments for new PACE enrollees and semi-annual re-assessments as needed. The RN performs general and skilled nursing care, implements the plan of care and physician orders, observes and reports participant conditions and reactions, and triages participants to determine priority of care. This role also participates in nursing on-call rotation for after-hours assistance, receives and responds to participant complaints, reviews and signs off on orders, and disseminates instructions. Care coordination with outside providers and facilities, including hospitals, nursing homes, assisted living facilities, and physician offices, is a key responsibility. The RN provides treatment instructions and nursing education to participants and families, assists with clinic and exam room maintenance, and maintains high customer service standards. They manage and review participant records, ensuring compliance with local, State, and Federal laws and regulations. The RN establishes productive working relationships with contracted providers, adheres to program cultural standards, and ensures adherence to organizational policies, procedures, and safety guidelines. They educate staff on elderly care as needed, act within their scope of practice, handle infectious specimens with appropriate precautions, and maintain confidentiality. Participation in continuing education and training meetings is required.
