About The Position

Amedisys is hiring a Registered Nurse case manager for a home health role. The schedule is seven days on and seven days off, Wednesday through Tuesday, 8 a.m. to 5 p.m. The nurse helps patients heal at home by assessing their needs, coordinating and implementing care plans, supporting patient health and independence, and communicating with the care team. The position also supervises LPNs and home health aides, completes timely and accurate documentation, provides staff education, participates in continuing education, and performs assigned on-call duties. Annual pay is $73,000 to $82,000, with compensation varying by geographic market and job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states where the employee is assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on patient needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
