About The Position

Western North Carolina Community Health Services (WNCCHS) is seeking a full-time Registered Nurse (RN) to join their growing team. This position focuses on delivering patient-centered, team-based primary care to residents of Western North Carolina, with an emphasis on advancing health equity. The RN will provide coverage for the McDowell Health Center location.

Requirements

  • Current license to practice as a Registered Nurse in North Carolina
  • Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN)
  • Strong communication skills
  • Strong clinical assessment skills
  • Proficiency in electronic medical records (EMR)
  • Ability to work in a team-based environment
  • BLS /CPR current certification required

Nice To Haves

  • Proficient in Microsoft Office programs
  • Minimum of two (2) years’ experience in an outpatient setting
  • Familiarity with role of FQHC in the community
  • Bilingual in Spanish preferred

Responsibilities

  • Deliver comprehensive, patient-centered care in collaboration with the healthcare team, focusing on health promotion, disease prevention, and chronic illness management.
  • Conduct patient assessments, administer vaccinations and routine medications, manage chronic conditions, perform screenings, and provide patient education.
  • Assist with walk-in and phone triage.
  • Conduct physical exams, measure vital signs, perform wound care, ear syringing, and administer immunizations/injections.
  • Complete co-visits with medical providers.
  • Monitor and manage chronic conditions such as diabetes, asthma, and hypertension, including medication adjustments based on protocols, and counseling.
  • Provide guidance on healthy lifestyles, managing illnesses, and preventive health measures.
  • Document medical histories, manage patient records, and coordinate care with other specialists.
  • Provide over-the-phone and in-person triage to assess patient needs.
  • Participate and/or lead quality improvement activities.
  • Maintain current knowledge of clinical best practices, guidelines, and evidence-based standards, and integrate them into daily nursing practice.
