Concierge Home Care is seeking a Weekend Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes, ensuring continuity of care during the critical weekend window. Requirements

• Active, unencumbered Florida RN license.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conducting patient assessments and developing individualized care plans.
• Providing skilled nursing care and essential patient/caregiver education.
• Coordinating care with physicians and interdisciplinary teams.
• Monitoring progress and adjusting care for patients recovering at home.
• Documenting visits efficiently using our streamlined AI-assisted tools.
