About The Position

Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, is the second largest multi-specialty group in New Hampshire, serving the greater Nashua community. Our Immediate Care service offers a quick, walk-in option for patients of all ages with non-life-threatening conditions, open seven days a week, including evenings, with an average wait time of 15 minutes or less. We maintain close communication with patients' primary care physicians.

Requirements

  • Current NH Registered Nursing (RN) licensure is required.
  • Current BLS/CPR training.
  • Prior work experience in a healthcare setting is required.

Nice To Haves

  • Epic experience, is a plus!

Responsibilities

  • Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.
  • Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities.
  • Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.
  • Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
  • Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
  • Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
