About The Position

Interim HealthCare is hiring a Home Health Registered Nurse to provide one-on-one, home-based care to patients in Beloit, Wisconsin, and surrounding areas. Patients may have illnesses, injuries, or chronic diseases and often need care after discharge from a hospital, rehabilitation center, or nursing facility. The role offers flexible assignments, autonomy, and the opportunity to direct your own day while working with a team of physicians and other care providers.

Requirements

• Graduate of an accredited nursing program.
• Active RN license in Wisconsin.
• At least 1 year of nursing experience; home healthcare experience is preferred.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in a nursing specialty.
• Strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Work with physicians and oversee LPNs, CNAs, and aides providing patient care.
• Examine patients, conduct assessments, document progress, and report changes to physicians.
• Educate patients about their plan of care, prescribed medications, therapy, diet, and exercise.
• Coach family members on the patient’s care plan, medications, and home safety.
• Ensure patient goals are met and coordinate discharge from services.
