About The Position
At Touching Hearts at Home, we help older adults remain safe, comfortable, and independent in their homes. We are seeking compassionate Registered Nurses to support our caregivers and clients throughout the greater Charlotte area and surrounding counties. This role is ideal for nurses who enjoy building meaningful relationships, educating caregivers, and improving quality of life without the stress of bedside ratios or shift-heavy schedules. Assignments are territory-based and scheduled as close to your home as possible.
Requirements
- Active North Carolina RN license
- Current CPR/BLS certification
- Strong communication and interpersonal skills
- Reliable transportation and willingness to travel within assigned territory
Nice To Haves
- Home care, geriatrics, hospice, or community-based nursing experience
Responsibilities
- Conduct in-home assessments and develop individualized service plans
- Perform supervisory visits to ensure caregiver performance and client satisfaction
- Train and support caregivers on care plan implementation and best practices
- Monitor client wellbeing and communicate changes or concerns appropriately
- Provide education and guidance to clients and families
- Maintain timely and accurate documentation
- Serve as a trusted resource and advocate for clients and caregivers
