About The Position
BAYADA Home Health Care is seeking a Registered Nurse (RN) for a full-time opportunity with our new Central NJ Senior Living team, a Medicare-certified home health office. This position provides one-on-one skilled nursing care to adult and geriatric clients living in assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities serving individuals with intellectual disabilities throughout Somerset, Middlesex, Monmouth, and Union Counties, NJ. As a BAYADA Registered Nurse, you'll have the opportunity to build meaningful relationships with your clients while providing high-quality clinical care in the place they call home. You'll work independently while being supported by an experienced interdisciplinary team that values your clinical expertise. This position may also include seeing clients in traditional home health community settings.
Requirements
- Current New Jersey Registered Nurse (RN) license.
- Minimum of one year of recent, verifiable clinical nursing experience.
- Graduate of an accredited and approved nursing program.
- Ability to work independently and effectively manage time.
- Strong communication, interpersonal, organizational, and clinical assessment skills.
- Reliable transportation and ability to travel throughout Somerset, Middlesex, Monmouth, and Union Counties.
Nice To Haves
- Medical-surgical experience preferred.
- Home health or senior living experience preferred, but not required.
- Experience with electronic medical records (EMR) preferred.
Responsibilities
- Provide skilled nursing visits to clients throughout designated senior living communities and territories.
- Complete comprehensive nursing assessments and develop individualized plans of care based on client needs.
- Administer medications, provide wound care, and perform physician-ordered treatments and procedures.
- Monitor client conditions and communicate changes to the Clinical Manager or Client Services Manager.
- Follow, execute, and accurately document physician orders.
- Provide case management and coordinate care with physicians, therapists, and other members of the interdisciplinary team.
- Educate clients and caregivers to promote health, safety, and independence.
- Complete timely and accurate clinical documentation using BAYADA's electronic medical record (EMR) system.
