About The Position

This individual is responsible for the daily clinical aspect functioning to meet all residents’ needs. The role involves medication supervision/administration, resident admissions, completing care plans, clinical system tracking (e.g., wounds, infection control), and discharges. The nurse will also provide health consultations, notify physicians and families of resident health changes, respond to emergencies, maintain inter-departmental communication, ensure resident confidentiality, and participate in safety/disaster planning and educational meetings.

Requirements

  • Registered Nurse’s License current, with no judgments against it, in the state in which he/she is practicing.
  • CPR certified.
  • Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.
  • Basic computer skills.
  • Ability to work as part of a team and handle multiple tasks safely and effectively.
  • 1+ years of experience in a skilled, sub-acute, or memory care setting.
  • An Associate’s Degree or equivalent from a two year college or technical school, or Two years related experience and/or training equivalent combination of education and applicable experience.

Responsibilities

  • Medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
  • Admissions of new residents to include all admission assessments and documentation.
  • Completing acute and long-term care plans or service plans.
  • Clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing.
  • Discharges, assessments, and clinical documentation as indicated.
  • Provide general consultation regarding health-related concerns within the community.
  • Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
  • Respond quickly to all emergencies.
  • Maintain close communication with all departments and staff members.
  • Maintain confidentiality of residents’ and prospective residents’ information.
  • Understand role in the Safety & Disaster Plan.
  • Provide educational meetings for residents and staff.
