About The Position
Elara Caring is hiring a Registered Nurse Home Health to provide compassionate nursing care in patients’ homes. The role involves assessing patient needs, delivering care in accordance with care plans and physician orders, monitoring changes in condition, and supporting safe, continuous, high-quality care. The nurse also protects patient and staff privacy under HIPAA and educates patients and families about care and safety.
Requirements
- Current state license as a Registered Nurse.
- Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
- At least 1 year of experience as a nurse in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
Nice To Haves
- Home Health experience is preferred, but not required.
Responsibilities
- Assess patients to determine their care needs.
- Complete initial and ongoing patient assessments.
- Protect patient and staff privacy in accordance with HIPAA.
- Provide nursing care according to the plan of care and physician orders.
- Monitor patient conditions and report changes to the care team.
- Educate patients and families on their care and safety.
- Take appropriate action when patient conditions change.
- Help ensure quality, safe, and continuous patient care.
