About The Position
Home Services Unlimited is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide in-home care throughout the Indianapolis area. This position offers autonomy, flexibility, and the opportunity to build lasting relationships with patients while managing your own caseload. This role is for a Medicare-certified home health agency.
Requirements
- Active Indiana RN license
- ASN or BSN from an accredited nursing program
- Strong clinical, communication, and organizational skills
- Ability to work independently in a home health setting
Nice To Haves
- Home health experience preferred, including OASIS
- Experience with electronic health records (EHR) preferred
Responsibilities
- Perform comprehensive in-home patient assessments (including OASIS)
- Develop, implement, and evaluate individualized care plans
- Provide skilled nursing care and treatments
- Educate patients and families on care plans and disease management
- Coordinate care with physicians and interdisciplinary team members
- Supervise LPNs and ancillary staff as needed
- Complete timely and accurate electronic documentation
- Ensure compliance with all regulatory and agency standards
- Participate in team meetings and quality improvement (QAPI)
