About The Position

ComForCare Home Care is seeking a skilled, compassionate, and dependable Registered Nurse (RN) to provide home health services to patients throughout San Diego County. This flexible role is available on a per diem, part-time, or full-time basis, depending on candidate availability and business needs. This position is ideal for an RN who is confident completing Start of Care assessments and providing ongoing skilled nursing visits in the home setting. You will play an important role in helping patients remain safe, supported, and as independent as possible in the comfort of their homes. We are looking for a clinician who brings strong assessment skills, sound clinical judgment, professionalism, and a patient-centered approach to care.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in California.
  • Current CPR certification.
  • Strong clinical assessment, communication, critical thinking, and documentation skills.
  • Ability to work independently, manage time effectively, and prioritize visits in a field-based setting.
  • Strong verbal and interpersonal communication skills with patients, families, physicians, and care team members.
  • Commitment to HIPAA compliance and patient confidentiality.
  • Valid California driver’s license, reliable transportation, and current auto insurance.

Nice To Haves

  • At least one (1) year of nursing experience in home health.
  • Experience completing Start of Care assessments and ongoing home health skilled nursing visits.
  • Working knowledge of OASIS, care planning, and home health documentation requirements.
  • Experience with wound care, chronic disease management, medication teaching, and patient/caregiver education.

Responsibilities

  • Perform Start of Care, comprehensive, and ongoing skilled nursing assessments in patients’ homes throughout San Diego County.
  • Complete timely and accurate clinical documentation, including OASIS and other required home health documentation, in accordance with agency, payer, and regulatory requirements.
  • Develop, implement, and update individualized plans of care based on physician orders, patient needs, and clinical findings.
  • Provide skilled nursing services within scope of practice, including medication management, wound care, patient assessment, disease management, and other ordered nursing interventions.
  • Identify changes in patient condition and communicate significant findings promptly to the physician, clinical leadership, and interdisciplinary care team.
  • Educate patients and caregivers regarding diagnoses, medications, treatment plans, self-management, safety precautions, and disease prevention.
  • Assess the home environment for safety risks and recommend interventions to support safe care at home.
  • Coordinate care effectively with physicians, case managers, therapists, caregivers, and family members to support quality outcomes.
  • Follow infection control, patient privacy, documentation, and safety standards at all times.
  • Participate in care coordination, case conference discussions, and other clinical communications as needed.
