About The Position

Legacy Hospice is seeking a Per Diem Hospice RN Case Manager to provide end-of-life care throughout Richmond and surrounding counties. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families through their hospice journey as part of an interdisciplinary care team. Care is centered on compassion, clinical excellence, patient comfort, dignity, and connection. The schedule is weekdays, per diem, and the position shares call duties.

Requirements

Current, unencumbered Registered Nurse license.
At least one year of RN experience in a medical-surgical or acute-care setting.
Computer proficiency, including typing, use of various programs, and experience with electronic medical records.
Current CPR certification if required by the state.
Current driver's license and vehicle insurance, with access to dependable transportation.

Responsibilities

Respond to clinical referral information, inquiries, and requests with care and expertise.
Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
Coordinate care and provide direct nursing support to patients, prioritizing comfort and quality of life.
Deliver nursing care using proven principles and techniques to ease pain and provide comfort.
Educate and advocate for patients and families, encouraging their participation in personalized care plans.
Share responsibility for call duties.
