About The Position

Adoration Health is seeking a dedicated Home Health Registered Nurse to provide patient-centered care in Cleveland, Mississippi, with coverage in Bolivar and Northern Sunflower. The position offers a PRN schedule with flexible scheduling. Home health and HCHB experience are preferred.

The RN provides one-to-one care in patients’ homes, assesses and monitors their needs, develops care plans in alignment with physician orders and patient goals, and delivers nursing interventions. The role includes supporting patients’ families and caregivers, collaborating with an interdisciplinary team, documenting care, and participating in the local branch’s on-call rotation as required. Nurses work independently with team support and may benefit from greater work-life balance and less time on their feet than in other settings.

Requirements

• Associate or bachelor’s degree in nursing from an accredited college of nursing.
• Current unrestricted nursing registration and license in the state of practice and state of residence.
• At least one year of nursing practice in a patient care setting.
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
• Current CPR certification.

Nice To Haves

• Home health, geriatrics, or other related-setting experience.
• Home health and HCHB experience.
• New nursing graduates may be considered in select markets, based on program availability.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create home health care plans aligned with physician orders and patient goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the local branch’s on-call rotation as required.
