Internal/External Registered Nurse -Geriatrics Team
About The Position
The Registered Nurse will be part of the Geriatrics Team at the Sarnia Site. This is a temporary full-time position for an existing vacancy. The role involves providing geriatric specific assessments and treatment planning, liaising with other caregivers for continuity of care, supporting treatment plan implementation, and providing crisis prevention and intervention planning to prevent relapse and/or hospitalization. The nurse will also develop partnerships with long-term care (LTC) homes, provide education to care providers in LTC, and act as a resource to CMHA staff and other service providers in Lambton County.
Requirements
- Bachelor of Science in Nursing degree
- Current registration with the Ontario College of Nurses
- Valid Ontario Driver’s Licence
- Access to a vehicle
Nice To Haves
- Canadian Certification in Psychiatric/Mental Health Nursing
- Proficiency in both official languages
Responsibilities
- Providing geriatric specific assessments and treatment planning
