About The Position

Interim HealthCare is hiring a Home Health Registered Nurse to provide one-on-one, home-based care to patients in Rockford, Illinois, and surrounding areas. Nurses have flexible assignments, autonomy in directing their workday, and coordinate care with physicians and other care team members. The position offers a base salary of $80,000–$95,000, with potential bonuses each pay period.

Requirements

• Graduate of an accredited nursing program.
• Active Registered Nurse license in Illinois.
• At least one year of nursing experience; home healthcare experience is ideal.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in a nursing specialty, strong communication skills, and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases, often following discharge from a hospital, rehabilitation center, or nursing facility.
• Work with physicians and oversee LPNs, CNAs, and aides providing patient care.
• Examine patients, conduct assessments, document progress, and report changes to physicians.
• Educate patients about their care plans, prescribed medications, therapy, diet, and exercise.
• Coach family members on the patient’s care plan, medication, and home safety.
• Ensure care goals are met and coordinate discharge from services.
