Requirements

  • Baccalaureate Degree BSN, ADN Degree, or Diploma RN. Major: Registered Nurse.
  • RN with a minimum of 1 year of RN experience.
  • Current BLS certification.
  • Proof of legal authority to work in the United States on the first day of employment.

Nice To Haves

  • ACLS certification or obtain certification within one year of hire date.
  • Knowledge or experience in PCMH desired.

Responsibilities

  • Performs comprehensive nursing assessments and identify patient health needs.
  • Develops and implement individualized care plans in collaboration with physicians, nurse practitioners, and other healthcare providers.
  • Coordinates patient care, including referrals, follow-up appointments, diagnostic testing, and continuity of care.
  • Educates patients and families on disease prevention, health maintenance, medications, treatment plans, and self-management strategies.
  • Manages prescription refill requests according to clinic policies and provider orders.
  • Monitors patient progress, document assessments and interventions in the electronic medical record, and communicate changes in patient status to providers.
  • Serves as a clinical resource for the Women's Center by developing expertise in women's health conditions and treatments.
  • Collaborates with an interdisciplinary team, including physicians, advanced practice providers, social workers, pharmacists, and other healthcare professionals.
  • Supports quality improvement initiatives and implement evidence-based practices to improve patient outcomes.
  • Provides compassionate, patient-centered care while maintaining patient privacy, safety, and regulatory compliance.
  • Assists with clinic procedures and ensure appropriate patient preparation and follow-up as needed.
  • May perform other duties as assigned
