Requirements

Baccalaureate Degree BSN, ADN Degree, or Diploma RN. Major: Registered Nurse.
RN with a minimum of 1 year of RN experience.
Current BLS certification.
Proof of legal authority to work in the United States on the first day of employment.

Nice To Haves

ACLS certification or obtain certification within one year of hire date.
Knowledge or experience in PCMH desired.

Responsibilities

Performs comprehensive nursing assessments and identify patient health needs.
Develops and implement individualized care plans in collaboration with physicians, nurse practitioners, and other healthcare providers.
Coordinates patient care, including referrals, follow-up appointments, diagnostic testing, and continuity of care.
Educates patients and families on disease prevention, health maintenance, medications, treatment plans, and self-management strategies.
Manages prescription refill requests according to clinic policies and provider orders.
Monitors patient progress, document assessments and interventions in the electronic medical record, and communicate changes in patient status to providers.
Serves as a clinical resource for the Women's Center by developing expertise in women's health conditions and treatments.
Collaborates with an interdisciplinary team, including physicians, advanced practice providers, social workers, pharmacists, and other healthcare professionals.
Supports quality improvement initiatives and implement evidence-based practices to improve patient outcomes.
Provides compassionate, patient-centered care while maintaining patient privacy, safety, and regulatory compliance.
Assists with clinic procedures and ensure appropriate patient preparation and follow-up as needed.
