About The Position

Elara Caring is hiring a Registered Nurse for home health care. The nurse provides compassionate care to patients in their homes, contributing to the delivery of the right care at the right time and in the right place. The role includes assessing patients and their care needs, providing nursing care according to plans of care and physician orders, monitoring patient conditions, communicating changes to the care team, educating patients and families, and supporting safe, continuous, high-quality care. Patient and staff privacy must be protected in accordance with HIPAA.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on their care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
